The SHD Max Plan is Smart Health Dental’s top-tier dental insurance. It pairs no waiting period or deductible on diagnostic and preventive services with 80% coverage on basic services after a 6-month waiting period and 60% coverage on major services and orthodontics after a 12-month waiting period, up to a $3,000 annual maximum.
What is the annual maximum on the Max Plan?
The Max Plan’s annual maximum is $3,000 — the most the plan will pay for covered services in a benefit year. That is more than three times the $750 maximum on the Preventive PPO MAC plan and is designed for members planning higher-cost basic, major, or orthodontic care.
Does the Max Plan cover orthodontics and braces?
Yes. The Max Plan covers orthodontics at 60% after a 12-month waiting period for dependent children up to age 19. There is a separate orthodontic lifetime maximum of $1,200; the most the plan will pay toward orthodontic services across the member’s lifetime,
regardless of the annual maximum.
Is the Max Plan available in every state?
